• The PC only has 1 HDMI port and 3 display ports.

• The video card has both HDMI and DisplayPort outputs, so you wouldn't be able to plug it in directly. You'd need an adapter to go from either an HDMI or DisplayPort cable t go to the VGA port (which, unless your monitor dates from the 1980s, is what you have)
