Yes it needs an internet connection to work. It doesn't "create" one. Internet doesn't work like that. It has to come from somewhere. This router just allows your connection to be spread out.

The router just simply takes that input and splits it to all devices you have connected in your house. So to answer your question: you do need an ISP, but your may not need their routers depending on the configuration you want in your house.
