“Is the 20" extension in addition to the 3.5" from the wall in the closed position? Or is it a total of 20 inches from the wall?”

Looks like 20" overall. But that gives lots of flexibility for pointing the screen where you want. I can get about 45 degrees of angle, with a 60" Samsung, when fully extended. ( Maybe a little more, I have a speaker in the way)
