It is on all the time. I use a Samsung phone (S8 edge) and am non-ambulatory with MS . I have the LTE watch, but not activated (treat as if Bluetooth watch). If your watch is within Bluetooth range (~20 without barriers) of your phone, watch is connected to wifi (same network as phone) or the watch is activated LTE (~$10/mo), a fall (which you setup via the Samsung Galaxy Wearable app on your phone/watch for LTE) will contact your emergency list via text message. I do not know about calling or to 911. The text message to your emergency contact list will send location (Google Maps link) and message for help every certain interval for 24 hours or until cancelled. A fall has a certain definition (check Samsung website for definition to prevent false-positives). Three presses of the Home button on the watch will act like a fall. For falls where the victim is conscious, able to work the watch and no emergency notification went out, I would advise sending a text message or call from the watch to one person to help. The emergency contact list may get annoyed if the fall is not cancelled. If the user does not have a phone or a phone within range, I would advise getting the LTE version and paying the extra per month.
