Q: Does the Insignia RF modulator output an NTSC (analog) signal to the coaxial cable? Thanks.

A: I bought this to hook up to my Vizio TV. The RF modulator did not work for me. The cable system is analog. I might have been doing something wrong, but I followed the instructions, tried channel 3 and 4, tried all the different inputs on the tv, and every connection with the RCA cords. Still could not get it to work.

Q: My cable company forced me to "upgrade" my old DVR, but the new DVR is apparently not compatible with my old TV. The tech suggested I try an RF modulator. How exactly do I hook this up then?

A: I'm stuck on step 1. My cable box is the source component and it only has HDMI. My old TV does not have HDMI. Am I plugging in the RF modulator to one of the other TV inputs having red/white/yellow input?

I would like to begin by clarifying that, by the description of the setup you have at home, this device would be of no use for you, as your TV is most likely capable of receiving RF signals via Coaxial cable.
