Q: Does the surge protector have an on/off button? (Not seeing one in the images.)
A: Yes power switch is on the left side if you are reading the "protected" light (near the end where the cord enters). It is recessed so that it can't accidentally be turned off. But it might be less accessible if you were to have it on its bottom, against a wall. I have mine located with the switch up.
Q: Does both indicator lights supposed to light up. mine only had grounded light on.
A: Yes both lights on means everything is good. Once one light is off replace the surge protector
