Assuming that you set the clock (it would probably ask you to), the other reason would be that you are taking an action that cancels the timer. In the final minute there is a countdown on screen. Anything that makes the countdown box go away cancels the timer, e.g. changing channels, volume, etc... otherwise it's malfunctioning.
