There is no monthly fee for Apple TV. There is always a monthly fee for Netflix and Hulu and other shows or movies you want to rent.

The monthly fee is only for subscriptions. You can stream you own music, movies or tv shows through your appletv from your computer. That have to be in iTunes though and iTunes has to be open on you computer. If you already have an account for Hulu and Netflix you can login on the apps on the appletv.
