I believe if you want calls you have to have it set up through your phone service which is costly. Otherwise you just get messages. I just get messages and bought the watch outright otherwise you will pay a monthly fee.

For the Apple Series 6 Watch, I set the watch app on my phone to mirror the notifications on the watch.
