• Yes it does! But it’s very simple I think it was 2-3 pieces that you just push and click into each other that’s all took less than a minute !

• Yes it does require assembly, but it’s very easy. I assembled it in 5 minutes.

• Yes, the only assembly is putting together the fan and the base
