I measure after the TV is attached with it's part the wall mount panel. Then I add 6" because I like mine mounted up and then tilted down about 5 Degrees. It really depends on what your viewing angle is.

Get to the place you plan on watching it from most and have a seat or get in the position you plan on watching it from. That focal point you are staring at is probably the place you want to put it. The mount itself has some adjustability.
