Press it once and the mouse moves by clicks. Press it again, and it flies( scrolls fast). I use both, depends upon what I am doing.

In "click" mode, each click of the wheel moves 3 lines on screen (by default). In smooth spinning mode, you can push the wheel quickly, then keep your finger off it and it will continue spinning freely. Click mode is nice for precision movement and free spinning is great for massive documents or pages that you need to scroll through quickly.
