If you buy 3 years, it is about $100 (after tax). One year, I think, is $40 But I would strongly suggest NOT buying this piece of garbage. Check out other review sites and you'll see this Magic Jack thing gets 1 star. I learned the hard way

It depends on which version of magicJack you purchase. The most current version is roughly $4 per month. You may also get a charge for 911 service and this is between $2 and $3 per month. This still works our considerably less than the phone or cable companies. There also are some other nice features. You can take your magicJack with you outside of the US and not be charged for calls to the US.
