No you don't have to cancel the subscription! If you use the gift card it will add the 3 months to your current subscription. For example, if you have 5 months of xbox live left and then use your 3 month gift card you will now have 8 months of xbox live.

No the 3 month card will extend his services by the 3 months you would be charged again the monthly rate after the 3 month period is up. You can always go to account.microsoft.com and select subscriptions in order to see when that would he and make changes to your account.
