• You do not need a separate attachment, but it makes iced coffee, so you put a full glass of ice and use the ice coffee feature. If you want "frozen", you would need to then pour it into a blender and blend.

• Yes. You would need a blender. This only makes the coffee. It will supposedly brew the coffee stronger for frozen coffee drinks, so that it is not diluted by the ice.
