“Is this device "protect or disconnect"? I assume the surge protection on this device is a one-time use (it cannot be reset). If so, once it is tripped, does it cut off power completely, or does it continue to provide power, but unprotected? I am looking for something that will cut off power completely if it is no longer providing surge protection. Thanks.”

According to the directions it resets itself. It states that as long as the blue light is on, on the front it is in protection mode. Mine has tripped twice so far.
