It appears to be by device for the promo. So two devices, two promo rewards. You go through the Shop Samsung app on the phone itself unless you ordered more than 5 devices. Samsung locks you out from computer registration if just a single device. Just make sure you have a picture or scan of the receipt to upload with your claim. When I got mine we bought two at the same time, one receipt. So far, both have been accepted for the promo.
