One monitor can portrait through this device. One monitor can portrait through a port on the laptop. The additional HDMI port will not allow it. (Don't know about MBP2018 specifically, I'm on 2020 MBP M1

You should be able to, but you'll have to adjust the display in your system preferences to reflect the angle you want.
