A gift card you can use towards the purchase of whatever is sold on the PSN store. Movies, games etc... The 12 month membership is a subscription that gives you access to free monthly games and discounts throughout the length of your subscription on the PSN store.

The plus card adds money to your account but if you have to extend your membership, it will cost the full price. Membership card is better for membership extension if you get the card on sale.
