“Why does the Japan and U.K. version of the console come with a download code or the game while the marketing on U.S. sites list "Game not included"? What's the deal?”

Because the other countries also raised the price of the console to include the game's price. So if the game were included in the US, the bundle would be priced at $359.99 (Switch + game cost). Nintendo of America did the consumer-friendly thing by NOT bundling them and allowing those who prefer physical games to purchase it that way.
Sorry, there was a problem. Please try again later.Another reason why they aren't including it as a bundle in the U.S is that the game comes out on a separate date than the switch. New Horizons comes out on the 20th while the switch comes out on the 13th.
