I don't have a PS4 slim but I have a regular playstation 4 and I just hooked it up to a USB port on the playstation and then went through the Playstation settings and reformatted the hard drive for the playstation

I have the same model for my PS4, after formatting it to the system and changing the game storage setting, it works great!
