You do not have to open both doors to access the inside. You can only open one door if that is what you want to do. I love the feature of being able to get filtered water from the inside of the fridge. I guess how much you use that feature and how much ice you use is going to determine how often to change the filter (and what your water quality is like).

We recommend replacing the filter every 6 months (or 200 gallons). We have found that an average household will deplete the 200 gallon capacity in about 6 months time. Contaminant reduction declines after exceeding 200 gallons even though the taste might not immediately change. However, the time frame may be different if water is consumed more.
