“I have a 1/4" to 3.5mm adapter hooked up to a pair of headphones and there's lots of distortion. It sounds normal when the plug is in halfway, but it's very loose and unpractical. I tested the adapter on other devices and it's fine.”

I am not an expert, but it sounds like you have a stereo plug and you are plugging it in to a mono adapter. I think you have to have the same type of plugs. If it works ok when you put it in half way, I assume you can only hear it on one side only.
