There is a gap between the screen protector and the case but it is only about a millimeter or two.

No, it is very easy to apply to your phone. But just like everything else, you have to be careful putting it on properly.

There is a small gap at the top and bottom, and little on the sides too. I also have the EVO case for extra protection. No issues.

It depends which case you have. Some cases over more protection, whereas, others may leave a gap.
