“Can this screen out sounds in the user’s surrounding environment such that the other party can’t hear them and will hear only the user’s voice. I’m a psychiatrist doing patient calls (Zoom & telephone). Sometimes my wife needs to enter the room briefly.”

No one can hear sound coming from them and no outside noise is heard by others in the call or zoom. My dog was barking and no one heard her. My husband and I share an office and he couldn't heard the entire conversation only what I was saying.

Depending upon the volume of your device settings, there shouldn't be any sound leakage for third parties to hear what's being said; however, there is no particular setting to mitigate against this from happening other than the standard volume control on your device.
