You pay both - plan and phone- as far as I am aware.

The carrier ( Verizon, ATT, Sprint,) has the phone base plans for the service and the you purchase then the phone is on a lease or buy plan. My phone plan was 40$ per month and the phone was buy one get one free, so one is 29.00 per month and the other is free. The bill the first couple months is screwy but then it works out.
