“Has anyone had a problem where they ONLY hear audio when connected to the iPad and zero video??”

• I did have one instance of picture but no audio but not what you are asking about. The problem occurred in a hospital where I had to plug into a wall connection, not directly into the TV. I assumed the problem was with that connection as it worked with all other TVs I have tried it on. Suggest trying either a different TV, or different HDMI cable.
