If your warranty is Apple Care, contact Apple. If it is a Best Buy Warranty contact Geek Squad. Might I suggest after it is repaired you get a "Hard Glass" (Brand Name) or equivalent screen protector? It is well worth the cost.

If you purchased AppleCare then go directly to an Apple Store. With AppleCare the Apple Store can look up your warranty by the product serial number - hassle free!!! If you purchased Geek Squad then go to BestBuy.

Since your warranty was paid to Apple, I would expect them to handle the claim.
