“I am thinking of switching to Century Link for internet. They require a DSL phone jack to connect to the modem, not a cable jack. Will this work? If not, any suggestions? Thanks!”

• My understanding is that this is for cable based internet only (DOCSIS). Ask your new provider if this would be sufficient. Most providers have a listing of compatible modems. You would just need to give them the make and model number.
