You can unlock the door “buzz them in” but then re-lock the door once they are in. When it’s time to leave they can manually unlock the door or you can do it electronically. Then again it must be re-locked either manually or electronically. Hope this helps. Sounds like you want a system that isn’t a dead bolt.

I believe you'd be able to set up some automation that when the door is shut then it would automatically lock the door. The patient when leaving can just rotate the lock manually to get out. It's really simple, then the automated task should be able to lock the door behind them. I honestly for a setting such as yours would look more into something commercial like most hospitals and child daycares have. It would be more expensive but much more professional.
