- Answer Contact Belkin customer support. Had the same issue with mine. They just need a copy of the receipt and a picture of the adapter. I called their support number. It took awhile to get through about 20 mins so make sure you have time. But once I got through they were very helpful. I had the replacement a week later.

- Answer Either way you plug it in. Only port is the line to the phone that is dedicated. The other two can be switched with no issues.
