“The water in the water tank for this espresso machine becomes cloudy and has floaty debris as though the milk from the frother has entered back inside the tank. Is this a design flaw? Do others have the same experience?”

not going to lie I have stopped using the milk frother for this very reason. the black part came off the milk fother after about 2 uses for me and after expelling the built up pressure thought it multiple times the metal produces some sort of outside layer of white setiment looking mateiral on the inside of the milk frother housing.
