“Do you have to have cable or stream via youtube, hulu, etc. to get tv shows or can you only get the over-the-air shows? I'm looking to ditch cable but don't know what to expect if I get this instead. I have Amazon Prime but want to watch different shows”

You can use this for OTA shows with just an antenna. To use the apps such as Hulu, YouTube and Amazon Prime, you would need to have an internet connection of some sorts to the Tivo, which typically comes over cable. You should be able to subscribe to just internet from the cable company without.

I leased the CableCard from Verizon for $5.99/month, turned in the set top boxes. Have Amazin Prime and Netflix. The combination works for me. I don't try to get over-the-air tv. Haven't tried Hulu or Vudu.
