I'll assume Tivo, pardon me if I guessed wrong. :) The unit itself is a one-time purchase. But you need to buy the SERVICE from Tivo. This is essential, as it provides the cable guide information: what shows are on at what times, and so on. You need this to schedule recordings for all episodes of The Simpsons, for instance. You buy the service directly from Tivo. You can get it for $14.99/month (which is cheaper than the $20 Spectrum charges here in Florida), or maybe $150/year. I chose the lifetime option of $550, which covers the unit for IT'S lifetime. This pays for itself in under 4 years. If you plan on having it that long and can afford it, I'd recommend that route. If you meant the Netgear cablemodem/router, there is no monthly fee other than your cable's internet charge. (Thought I'd throw that in just in case. :) )
