No it doesn't come with a mat or any other accessories. It will move so I put mine on a rubber backed bath rug and it stays firmly in place.

No matt, it comes with chair leg holders to keep your chair from shifting while you pedal.

No....it does not. But the mat is highly recommended to keep the unit from moving and putting undue pressure on you inner thigh/groin area if you feel that you are stretching to reach the unit when it starts to slide.
