In regards to the 20 + 4 pin connector not having one hole filled in, yes this is normal, if you look at the supplied pinout picture, pin 20 is labelled as 'N/C' which means No Connection and is intentionally left disconnected. As for the hole with 2 wires going into it, this is also normal, it is present on all the power supplies I own. Looking closely, it looks like the two wires are going into pin 13 which is labeled as +3.3V. If I had to guess the reason for this, I would say the wires are coming from different rails of the power supply, which helps distribute the load on the power supply and cables to prevent one cable from carrying the whole load.
