The code came with my game inside the disc case. Entered it on PSN store and had no issues

There is only one code in the case and it should say, "Croc Hunter Pack." Once you redeem that, you will receive an email with an activation code for any additional content. If you are missing the leaflet with code, please contact Best Buy Support so that they can replace the code for you.
