“Is HR the only additional feature this watch has over the sapphire with additional bands?”

• The fēnix 3 HR has several added features above and beyond the built-in heart rate sensor. It also offers courses, Auto Lap, Auto Scroll, time/distance alerts, compatibility with power meters, and area calculation. The fēnix 3 HR also has some physical differences when compared to the Sapphire models. (E.g., the bands are made out of different material, battery life is different, weight, etc...) -JPK

• The HR monitoring sensor isn't the only upgrade from the Fenix 3 Sapphire, you also get a better/bigger battery, and additional workout profiles I know Golf, and Stand up paddle boarding are a few I know right off the top of my head. The rest is merely aesthetics. They both have the same titanium color, however the silicone band the HR comes with is slightly different and in my opinion a little nicer looking than that of the Sapphire. Also, the HR does NOT come with the additional metal band that the Sapphire comes with.
