It'll work on whichever account you claim it on... If You have a main account with sub-accounts they can also download it and play it... The max is 2 consoles...

No, it is only one code that can be redeemed on your PSN account. You cannot redeem it as two separate versions of the game. You can install it on both your PS4 and PS5 if you have both but it will only work on whichever system your licensed account is currently logged into.
