Sorry, there was a problem. Please try again later.Here is how to do it. You will first need to upgrade the disc version digitally to the PlayStation 5 version. This will require a $10 upgrade patch in this case. Once you do that, you can use the same file to keep playing from the same point with the PlayStation 5 version of the game.
