No: you ONLY pay the monthly fee.

You pay a monthy fee depending on usge and that includes the cost of the cartridges.

There are 3 basic plans for HP Instant Ink.  With this program you pay for pages printed per month, not ink consumed.  The 50 page plan costs $2.99 per month, the 100 page plan costs $4.99  per month and the 300 page plan costs $9.99 per month.  There is tax added to that, but it is totally inclusive of mailing the cartridges, returning used cartridges and shipping.  If you go over your number of plan pages sets of 10 additional pages are $1.  If you don't use your plan, the unused pages roll over to the next month.  The largest amount of pages that can rollover in a month is twice the monthly page plan.   You may wish to check out this link to get more details:
