I had similar issue. Here was my fix: Use optical cable (not included) and in settings switch to tv internal speakers on. Now use tv remote to control volume. For Bluetooth use the remote included with sound bar.

If you have a setting called Audio Out, make sure it is set to variable. It seems like there were a couple of other settings I played with, but until I made that change, I wasn't getting any sound out with my RCA connections. I have since gotten it to work with both TVs.
Sorry, there was a problem. Please try again later.If you're using the digital audio out (also listed as optical), you need to go to your Smart TV's audio settings and select PCM audio output. If you are using the 3.5mm/RCA audio out, simply turn the TV's speakers down all the way because the volume control on the sound bars remote will not affect or control the TV. The TV's volume control has no affect on any external audio outputs on the TV. If you use a universal or programmable remote control (such as a Comcast/Xfinity remote, which I use), you can go through the programming codes and find one listed under Insignia (do not select an Insignia TV, instead, select the Insignia brand under Home Audio System or something to that effect), select one (I had to go through a few Insignia codes to find the right one) and from then on, it should control the sound bar functions such as volume up/down, mute and power.
