We just got this a couple weeks ago, so haven't learned everything about it yet. My wife loves it, though! I know you can adjust the intensity on the back, and the feet. I don't think you can adjust the calves. I also do not know about buying a different leg extension. It does a good job of sensing your body size and adjusting the length of the leg rests.
