“Bluetooth was disconnected from both the Meeting Mate and from the PC and the device won't connect via USB with Bluetooth turned off on both devices. Do I have to remove the device entirely from the PC's Bluetooth list of connections to run USB?”

First, I would make sure that it is configured correctly in the computer's settings, for Windows: Go to Start Menu > Control Panel > Sound, select Meeting Mate under the Recording and Sound Playback tabs, and choose Set Default.
