Hello. Those pointy little things are nestled into your ear as well as the overall size of the ear buds. You can adjust even further with the provided gels. I know what you mean and these things fit perfectly in my ear. I've ridden my bike, ran, sat, jumped, you name it. The ONLY problem, if you wanna call it that, is they do take some time to adapt to. They kind of hurt but you become accustomed to it within a week or two. Hope this helps.

For me personal and my ears I wear the large silicone tips and the medium wing tips and here are locked in and no amount of sweat or anything I am doing will allow them to just fall out. You have to find there perfect combination. And remember they will be tight in your ears and the less of the outside world you can hear the better they will sound.

Please try again later.It may not be obvious from the directions, but the little wing piece at the top of the bud actually goes into the curved cartilidge at the top above the ear canal. This produces a very snug fit[and somewhat uncomfortable, but I haven't gong through all the options yet]. Also using the foam cushions on the main part of the bud help produce a good seal, you have to pinch them before putting them in so that they expand when inserted.

Please try again later.From what I read this is the most common issue with these ear buds, since you have had a few operations I would maybe try the smallest and least invasive ear bud options just so it’s a little easier on your ear but there will be some minor discomfort if you keep them on for a long period of time. What I did honestly was spend a lot of time playing around with all the other pieces and seeing which one works best for me and put them through all sorts of tests like jumping jacks and opening my jaw as you did to see how long before it fell out or didn’t. So my advice would be to play around till you get a decent seal that works for you I hope that helped out a bit.
