• It gets incredibly loud...I don't know how you classify "extreme", but you need to not blow out your eardrum with it.

• The volume level is good. Not sure what you mean by "extreme" volume. There are volume controls on the headset and it also works on your device's volume controls.

• If, by extreme, you mean extremely bad, then yes. I've tried two different devices with my GS4 and neither are loud enough to be used in the car or outside.

• No, it doesn't. The earpiece is also a tad large and not all that comfortable in my ear.
