“I have the Extend A vent microwave. When I have the fan on and the vent extended, I feel the blower only from above. Am I supposed to feel a blower from below the extend a vent?”

No you won't feel the from below at all. The vent area is where the fan pulls the air into along with the fumes and the through filters and the air is blown back into the air at the front top of the microwave.
