Yes, The faceplate is included. There is one that you screw to the wall and then the outer one that snaps over and has the clean look that you see.

I bought two of these and mine did not come with a face plate. 02/2022

The PD-6WCL dimmer does not include the wallplate. This dimmer will fit within any standard decorator/designer wallplate. Lutron's example is the CW-1.
