As long as you have a Nano SIM you can just insert it into this phone, you may have to setup the APN settings if they don't update automatically however. Instructions for that may be on their website or you could go into the store for assistance.

Your moto g 3rd gen uses a different size sim card than the moto x, so yes, you'll have to get a new sim at US Cellular. As for contacts, everything is usually backed up to google by default so the apps you mentioned may not be necessary.
