The VPN works only with windows and iOS enabled devices. With the amazon firestick, its more so a plug and play thing so it wont give you that option if you are using an actual tv. However if the amazon firestick is connected to a computer instead, then you will be able to select an italy IP address and access the content. Tried it and it works.

No, this will not work without a device that has it installed that can be a hotspot or personal wifi. I have nordvpn installed on my laptop and with the laptops hotspot feature the devices can connect to the laptop that has nordvpn enabled and it will bypass the region restricted content. I have not tried this with the cellphones hotspot feature.

It gets good reviews but there is always the worry that you are letting the VPN service look at all of your internet activity.
